The Black Parade dropped on October 23, 2006. It's the story of "The Patient", a man dying of cancer, and death comes for him as his favorite memory: his father taking him to see a marching band. Its working title was The Rise and Fall of My Chemical Romance.

They made most of it with Rob Cavallo at the Paramour Mansion in Los Angeles, and living there took its toll, especially on Mikey. Welcome to the Black Parade started out as The Five of Us Are Dying, the hardest song on the record, and it only came together when Cavallo brought them the piano piece that opens it. Famous Last Words grew out of a Ray Toro idea called "The Saddest Music in the World", Frank wrote House of Wolves, and Liza Minnelli is on Mama. Teenagers is the one song that isn't part of the story.

Welcome to the Black Parade was their first UK number one single, and its number nine on the Hot 100 is still their only US top ten. The album debuted at number two in both countries, behind Hannah Montana and Rudebox of all things, and sold 240,000 copies in its first week in the US. Then in October 2026, after the 20th anniversary deluxe, it went to number one in the UK for the first time. Twenty years. No notes.

My Way Home Is Through You, Kill All Your Friends and Heaven Help Us are the B-sides, and Gerard's said he regrets leaving Kill All Your Friends off. The 10th anniversary reissue in September 2016 added a disc of demos called "Living with Ghosts", and the new songs from it are in this sort: The Five of Us Are Dying, Party at the End of the World, Not That Kind of Girl, Emily and All the Angels.
